Job Description:/h3:
GENERAL DESCRIPTION OF POSITION
Under the direct supervision of the Residential Manager, is responsible for the provision of support in regard to medical needs, personal assistance, training and quality care to people with intellectual disabilities residing in community group home(s).

PRIMARY JOB FUNCTION(S)
:Schedules and completes medical appointments for individuals served.
:Completes all necessary medical follow ups and required post medical appointments.
:Works with the pharmacy to ensure all medications are ordered and filled as prescribed.
:Completes medication reviews when new cycle medications are delivered.
:Trains Direct Care Professionals on all medical treatment protocols and procedures.
:Ensures adequate staffing for each home and reviews the staffing schedule weekly. Assists Residential Manager with finding coverage for vacant shifts.
:Provides personal care to persons served during specified time periods.
:Completes a household inventory on cleaning supplies monthly.
:Completes a food inventory weekly.
:Monitors documentation in the daily logs and van logs for accuracy.
:Review resident files weekly.
:Maintain organization of home offices.
:Be available to assist other Residential Managers for needed support.
:Provides direct support when individuals are home from their day program.
:Maintains individuals petty cash logs.
:Provides transportation to the individuals served when needed.
:Assists with emergency situations, communicates and documents such incidents in accordance with Agency policies and procedures.
:Complete Maintenance Request Forms as needed and reports issues to the Maintenance Technician.
:Works alongside Residential Manager to ensure the Emergency Food and Supplies Kit is sufficiently stocked.
:Works alongside Residential Manager to ensure the OSHA and First Aid Kits are sufficiently stocked.
:Uses technology for the completion of specified job duties.
:Attends work regularly according to assigned work schedule and in accordance with Agency policy.
:Attends and participates in in:service training, staff meetings and other activities to facilitate professional development.
:Works cooperatively with others including all staff, supervisors, administrators, co:workers, individuals served, community professionals, customers, vendors and the public.
:Assumes other duties, responsibilities and special projects as needed.
:Effective management skills with an ability to plan and implement systems that are consumer focused, outcome oriented, and ensures quality service delivery.

:Works cooperatively and effectively with stakeholders while meeting the needs of individuals served and the programs.

QUALIFICATIONS

:Ability to communicate on the telephone
:Valid drivers license from place of residence and ability to drive

:Ability to sit for long periods of time

:Ability to understand technical written material
:Compassion and respect for persons with developmental disabilities and the ability to earn their confidence
:Ability to lift 20 lbs.
:Ability to lift up to 50 lbs.
:Ability to lift more than 50 lbs.
:Ability to use computer
:Valid certifications in CPR/FA
:Full range of motion above the head
:Ability to bend, lift and carry
:Full awareness of environmental stimuli

EDUCATION
Associates of Arts or Science degree from an accredited college or university with emphasis in Human Services. Bachelors degree preferred. Must have transportation
